The LinkUp is a Bronx Health Center (BHC) telehealth program for monitoring chronic illnesses. We will provide you a small/portable, Internet, and video-enabled device to take home so we can communicate with each other. You will use this fun, easy-to-run unit and some simple devices to take your blood pressure, blood oxygen, and blood sugar readings daily so your doctor can monitor your health.

 

Your readings will be visible to you and also sent to our care team here at BHC for regular review. Our goal is to help you understand and manage your condition and symptoms. If you follow the program, you’ll feel better and have more energy.

 

We’ll be with you every step of the way, looking at your readings, sending you messages and quick surveys, and using this little device to have video chats with your doctor.
